Current petitions, forms and documents for undergraduate and
graduate students and academic staff of the Division of Art Education
Using Forms and Petitions
Students should download the current form or petition relevant to
their intended procedure from the list below and complete all
required fields. Depending on the nature of the application,
any required supporting documents should also be attached.
Completed documents should be submitted to the relevant academic
or administrative unit in accordance with the applicable regulations
and application procedures. Students who need clarification regarding
a procedure or its requirements are advised to contact their academic
advisor, the Division Secretariat, or the Faculty Student Affairs Office.
Important: Before submitting an application, please
make sure that you are using the current version of the relevant form
and that all required supporting documents have been included.
