Final registration procedures of the studdents who were entitled to receive education at our University via Vertical Jump Test (DGS) will be carried out by Pamukkale University Rectorate Student Affairs Department between 17-21 October 2016. Required documents are listed below.

Documents Required for Registration
1- Web printout of 2016 Vertical Jump Test (DGS) result document (1 Copy).
2- Original High School/Undergraduate Diploma taken from the school graduated or original Certificate of Graduation with a new date and the certified copy given by the graduated high school/university or a notarized copy. The original documents will be given back to the students during the reigistration procedures after seeing the original ones and only the certified or the notaized copy will be taken from the students.
(Students who will register with Provisional Certificate of Graduation will be required to get the original High School/Undergraduate Diploma from their schools. The original documents will be given back to the students after seeing the original documents and the certified copy given by the graduated high school/university will be submitted to Student Affairs Department).
3- The document showing the ID number (ID card copy).
4- 6 pieces of 4.5 cm x 6 cm size photos (Identification number, student number, name and surname will be written on the back of photos).
5- A document showing that there is no problem for the person to register a faculty or a college in terms of military service, which will be taken from the related military branch for male candidates who have reached the age of military service or older than that (for those who were born in 1994 and later).
6- The bank receipt showing the first installment of student contribution/tuition fee has been paid (for evening education students).
7- The committee report which will be taken from university hospitals and/ or a full-fledged official hospital and shows that the person has no physical and mental disability that will prevent him/her from receiving education or conducting the related job.
8- Student Information Survey (will be taken from the system during registration.)
9- Transcript of records will be submitted to the relevant academic unit with a certified copy after the registration (Original/certified by the relevant university).
10- Course contents will be submitted to the relevant academic unit with a certified copy after the registration (Original/certified by the relevant university).
